  • Children (We do not offer childrens vaccines in house, sorry for the inconvenience.)

• Narcotic addiction therapy: bupenorphine/suboxone (Both Dr. Welch manages in PCB and Vernon)

• Insurances: Rates, copays, and deductibles are dependent on your specific plan. Patients are responsible for services not covered by insurance. Self pay prices are always available, and may be applied in place of insurance, if requested before services are rendered. 

• We do not accept card payment at this time, however we do accept cash, and checks, (from banks other than regions) as payment. 

• Self-pay office visits (initial): $80 in Vernon and $100 in PCB

• Self-pay office visits (followup): $80 in Vernon and $100 in PCB

• DOT Physicals: $125 cash or check (Offered at PC and Vernon clinics.)

• Child school/sport physicals: $25 cash or check  (From banks other than regions)

• Procedures can be done for self-pay patients, please inquire about pricing at time of visit

• We no longer offer in house lab draws in PC and PCB clinics. 

• There are several imaging & lab draw centers in the region that have greatly discounted cash rates.

While we are able to accommodate the below insurances. Insurances change their policies periodically, and often, unpredictably. We strive for transparency in our prices, insurance companies do not. Consequently, we can not predict what individual plans will cover. Rates, copays, and deductibles are subject to your specific plan. Patients are responsible for services not covered by insurance.
